Gary,
Per our discussion is a copy of the ACH Form to be completed by the City
of Kalispell for their AIP 003-2010 grant. This form is for sponsors
that previously received paper checks for grant reimbursements. Future
payments will be direct deposit and therefore avoid the "paper" check.
The form can be e-mailed or faxed back to us. As soon as we receive the
form we can issue the grant. Thanks.
