Fire Station 61 - general contractor prequalification Request for QualificationsCity of Kalispell Post Office Box 1997 - Kalispell, Montana 59903-1997 Telephone (406) 758-7701 Fax - (406) 758-7758 REQUEST FOR QUALIFICATIONS GENERAL CONTRACT SERVICES DATE ISSUED: June 9, 2010 THE KALISPELL CITY FIRE DEPARTMENT for and on behalf of The City of Kalispell REQUEST QUALIFICATIONS FOR: Fire Station 61 Renovation Time and Date of Pre -Submittal Conference: 2:30 p.m. on June 15, 2010 Time and Date Set for Submittal: 5:00 PM on June 25, 2010 REQUEST FOR QUALIFICATIONS (RFQs) FOR GENERAL CONTRACTOR SERVICES FIRE STATION 61 RENOVATION INFORMATION AND INSTRUCTIONS TO GENERAL CONTRACTORS 1 GENERAL INFORMATION 1.1 The City of Kalispell intends to prequalify the General Contractor (GC) for construction and remodel of the Fire Station 61 located at 312 lst Avenue East, Kalispell. 1.2 The Statement of Qualifications (SOQ) must be received by the City Clerk at 201 First Avenue East, Kalispell, Montana, by 5:00pm local time, June 25, 2010. 1.3 Questions pertaining to the selection process should be directed to Dan Diehl, Fire Chief, at (406) 758-7762 or ddiehl o,kalispell.com. 1.4 The City of Kalispell shall not be held responsible for any oral instructions. Any changes to this Request for Qualifications (RFQ) will be in the form of an addendum, which will be furnished to all registered RFQ holders. 1.5 The City of Kalispell reserves the right to reject any or all SOQs, to waive any informality or irregularity in any SOQ received, and to be the sole judge of the merits of the respective SOQs received. The City will not reimburse proposers for any cost related to submission of these Statements of Qualifications. Firms are advised that any information, under State law, may be subject to public inspection. 1.6 A pre -submittal conference for the Project will be held at 201 First Avenue East, Kalispell, MT on June 15, 2010 at 2:30 p.m. City's architectural consultant and staff will discuss the scope of work, general contract issues and respond to questions from the attendees. Because staff will not be available to respond to individual inquires regarding the project scope outside of this pre -submittal conference, attendance at this pre -submittal conference is highly recommended. 1.7 The Project description is: renovation of existing fire station 61 located at 312 First Avenue East, Kalispell, Montana. 1.8 The prequalified bidders for GC will be selected on the basis of demonstrated competence and qualification for the type of services required without regard to fee, and thereafter the prequalified bidders will be invited to submit bids for the project. The City will then select the GC based upon the lowest responsive bid submitted. 1.9 The GC will be an integral member of the Project Team, consisting of the Architect/Engineer, General Contractor, representatives from the City, and other consultants as required. 2 - ANTICIPATED SCOPE OF WORK The project generally consists of the addition of a second story within the existing apparatus bay for the purpose of a day room, multi -purpose room, fitness area, sleeping quarters, and locker area. The existing quarters will be remodeled into office space with the addition of an elevator to serve a lower level secure reception area. The mechanical and electrical systems will be largely replaced in the affected work area as well. This project is funded by a fixed amount in the form of a Department of Homeland Security Grant. This grant has conditions and reporting requirements to which the selected contractor will be required to adhere. This project construction will entirely occur within built and currently occupied space. The Fire Department and other occupants of the Public Safety Building must remain in operation with as little disturbance to daily operations as possible. The facility is a 24/7 emergency responder facility and consideration must be given to this reality. Significant demolition and remodel will occur including structural items; successful experience in like -manner construction will be required. With a fixed budget, the City will have specific interest in the Contractors past performance on remodel projects of this nature with regards to change orders, schedule delays, and work quality. 3 - STATEMENT OF QUALIFICATIONS SELECTION CRITERIA 3.1 Firms interested in providing the services of the general contractor for the project must submit a Statement of Qualifications (SOQ) that addresses the following evaluation criteria. Applicants are encouraged to organize their submissions in such a way as to follow the general evaluation criteria listed below. Information included within the SOQ may be used to evaluate your firm as part of any criteria regardless of where that information is found within the SOQ. Information obtained from the SOQ and from any other relevant source may be used in the evaluation and selection process. 3.2 Cover Letter (1-page) containing at a minimum: Company name, contact name, address, fax number, and email address . 3.3 Qualifications Criteria 3.3.1 General Information Description of firm/team Legal company organization; organization chart with names List of applicable Montana licenses 3.3.2 Relevant Firm Experience (20 points) a. Applicant's overall reputation, service capabilities and quality as it relates to this project. b. List and briefly describe 3-5 comparable projects completed by your firm or currently in progress; include your firm's role, and discuss contract amendment history, if applicable. For each project, include: contract value and construction value (original value plus contract amendments, if applicable), project owner, project location, contact name and title, address, current/accurate telephone number, fax number, and email address (if available). c. A minimum of three referrals and references from other agencies and owners. If possible, references should be from the projects listed above. d. List and describe any litigation; arbitration; claims filed by your firm against any project owner as a result of a contract dispute; any claim filed against your firm; termination from a project. e. Applicant's capacity and intent to proceed without delay if selected for this work. 3.3.3 Team Experience & Qualifications (20 points) a. Describe each team member's position within the firm. Provide resumes of each proposed team member in Appendix A. b. Briefly describe each team member's role on this project. c. Provide "team" experience working together on similar projects. d. Identify proposed sub -consultants. 3.3.4 Project Understanding and Approach (20 points) a. Describe your understanding of the project. b. Identify and discuss any potential problems during construction. c. Identify and discuss methods to mitigate those problems. 3.3.5 Approach to Project Management (20 points) a. Describe your firm's project management approach and team organization during the construction phases. b. Describe systems used for planning, scheduling, estimating and managing construction services. c. Describe the firm's experience on quality assurance and dispute resolution. 3.3.6 Other Factors (20 points) a. Current workload and ability to proceed promptly. b. Willingness to abide by the City's standard form Agreements with few or no objections or changes. c. Provide statement regarding your assurance that this engagement will not result in a conflict of interest. d. Relevant factors impacting the quality and value of work. e. In an appendix, include the proof of the firm's ability to bond the project ($1.5m minimum), proof of the firm's general liability policy, as well as appropriate licensure. 4 - SUBMITTAL REQUIREMENTS 4.1 The SOQ shall include a one -page cover letter plus a maximum of fifteen (15) pages to address the SOQ criteria specified in Section 3 (excluding Resumes). Table of Contents and section divider pages do not count towards the total page count. Resumes for each key team member shall be limited to no more than two pages and shall be attached as Appendix A. 4.2 Five copies of the Statement of Qualifications and Appendix A must be submitted to the City Clerk at 201 First Avenue East, Kalispell, Montana, by 5:00 pm local time, June 25, 2010. 4.3 Failure to comply with the following criteria may be grounds for disqualifications: Receipt of submittal by the specified cut-off date and time. The number of originals and/or copies of the submittal specified. Adherence to maximum page requirements. 4.4 Adherence to the maximum page criteria is critical; each page side (maximum 8 1/2" x 11") with criteria information will be counted. Pages that have photos, charts and graphs will be counted towards the maximum number of pages. 5 - SELECTION PROCESS 5.1 A 4-member Project Evaluation Board will evaluate each Statement of Qualifications (SOQ) according to the above criteria and select up to five finalists who will be invited to bid the project. 5.2 Requests for debriefings or to review Statements of Qualifications submitted, shall be made in writing to the City. All information submitted by firms and related Project Evaluation Board evaluations and rankings shall be considered confidential until after contract execution and award by the City Council. 6 - ATTACHMENTS 6.1 Grant Documents 6.2 Plans and Drawings
