Re: $1,271,000 Sewerage System Revenue Bond (DNRC Water Pollution Control
State Revolving Loan Program), Series 2013

Dear Theresa:
In anticipation of the closing of the above referenced bond issue on Thursday, May 15,
2014, we have enclosed the following documents:
One blue backed bond which should be executed by the Mayor, City Manager,
and the City Finance Director on page 4 and sealed. The City Finance Director
should also execute the registration signature line prepared for signature on page
5. These are the only two pages that require signatures. Also enclosed is a
copy of the bond, marked "Specimen," for the City's files.
2. Four copies of the Officers' Certificate to be executed by the Mayor, City
Manager, and the City Finance Director and sealed.
3. Four copies of the Certificate Showing Due Execution of Construction Contract to
be executed by you and sealed.
Four copies of the Certificate and Receipt of Finance Director, to be executed by
the City Finance Director.
Four copies of the Certificate of Registrar, to be executed by the City Finance
Director.
6. One copy of the IRS Form 8038-G for the Mayor's signature plus an additional
copy marked "Taxpayer's Copy" for the City's file. We will file this form with the
Internal Revenue Service subsequent to closing.
We would appreciate it if you would return the original Bond and three copies of
documents 2 through 5, plus the originally signed IRS Form 8038-G, above, to our office for
receipt no later than Wednesday, May 14, 2014, for our review prior to closing.
