presenting this proposal, Lumen does not agree that the RFP, Lumen's response, or the City's contract template will
be a part of a final agreement in whole or in part.

identify the Lumen affiliate providing the services.
Critical 9-1-1 Circuits. The Federal Communications Commission's 9-1-1 reliability rules mandate the identification
and tagging of certain circuits or equivalent data paths that transport 9-1-1 calls and information ("9-1-1 Data") to
public safety answering points defined as Critical 911 Circuits in 47 C.F.R. Section 9.4(a)(5). Lumen policies require
tagging of any circuits or equivalent data paths used to transport 9-1-1 Data. We require that customers agree to
cooperate with Lumen regarding compliance with these rules and policies and to notify Lumen of all Services
customers purchase under the Agreement utilized as Critical 911 Circuits or for 9-1-1 Data.

Lumen is pleased to present this response to your Request for Proposal for Private Fiber and DIA Internet
Service.
Lumen is proposing to build dedicated fiber to each of the five (5) city facilities detailed in this Request for
Proposal to connect to our global intemet backbone. Lumen is proposing our "Dedicated Internet Access"
(DIA) service. Our DIA service is our carrier grade intemet service that provides enterprise Service Level
Agreements (SLA's) as part of the service. The 1 Gbps bandwidth to each site is a guaranteed upload
and download bandwidth. No part of the Lumen Dedicated Internet Access is shared with any other user
of the Lumen Network. The bandwidth is 100% dedicated to the City of Kalispell.
In addition, we included a quote for our Fiber+ / GPON Internet Services.
Lumen's service is supported by a dedicated account support team who assists the City of Kalispell with
the installation, activation, and on-going support of our services. Team Lumen looks forward to working
with the City of Kalispell on this project to improve the city's internet capacity.

1. Solution Overview

Lumen will install fiber optics into each of the City of Kalispell locations noted in this RFP to deliver our
Dedicated Internet Access service. Our Dedicated Internet Access is an enterprise grade internet service
and provides dedicated, direct connections between the City of Kalispell sites and Lumen's Global Internet
backbone. Lumen has dual fiber routes out of the Flathead Valley to provide the highest service availability
as backed up by our Service Level Agreements (SLAs).
Lumen provides local technical support along with our national Network Operations Centers (NOC) and
provides response times for outages of 2 hours based upon our service level agreements. Service issue
can be reported on-line in our Control Center customer portal or through a Toll -Free telephone number.

Lumen Response: Due to size of Lumen, various suits, proceedings, and claims typical for an enterprise
business can be pending against Lumen at any one time. While it is not possible to determine the ultimate
disposition and resolution of any suits, proceedings or claims, and whether they are consistent with Lumen's
position, Lumen expects the outcome of such proceedings, individually or in aggregate, will not have a
materially adverse effect on the financial condition or results of Lumen operations or its business segments;
nor negatively affect its ability to provide the services proposed.
d. Applicant's capacity and intent to proceed without delay if selected.
Lumen Response: As one of the few Tier 1 internet providers in the United States and the second most
peered global internet backbone, Lumen capacity is limited only by the needs of our customers. Today,
Lumen can deliver 100G Internet ports to the City of Kalispell if needed (soon to be increased to 400G
ports). Lumen's internet backbone connects directly to the major peering points, (Seattle and Denver for the
City of Kalispell) all on our own network as a single hop. Our internet traffic traverse only the Lumen
network between our customer's port and the closest peering location. This network design will provide the
City of Kalispell the lowest latency in the industry.
Lumen intends to proceed with constructing/installing the fiber as soon as awarded the business and a
contract is executed by both parties. Once the contract is signed, the project enters the Lumen Provisioning
Application and start the process of designing, permitting, and building the fiber to deliver services to the
City of Kalispell. Lumen foresees no delay in starting the project will proceed expeditiously.

4. Company Qualifications
a. Describe the company's history in the industry. Provide resumes of key personnel in Appendix A.
List professional continuing education.
Lumen Response: The company was incorporated in 1968 as Central Telephone and Electronics, drawing
on the assets of Oak Ridge Telephone Company, which had historical roots dating to the 1930s. In 1971,
the company was renamed Century Telephone Enterprises, Inc. and began trading on the New York Stock
Exchange in October 1978 under the symbol CTL. In 1999, shareholders voted to change the company's
name to CenturyTel, Inc. In 2010, the company formally began operating under the name CenturyLink, Inc.
In late 2020, the company renamed itself as Lumen, a multinational technology company providing the
fastest, most secure platform for applications and data to help businesses, government and communities
adopt emerging technologies and harness the power of the 4th Industrial Revolution. Accordingly, the
company began trading under the symbol LUMN on the New York Stock Exchange.

5. Contract Understanding and Approach
a Describe your understanding of the scope of work and the particular expertise your company
has in this area of work.
Lumen Response: Based upon the details in the RFP and the information provided by Erika Billiet at the
August 31St, Bidder Conference, Lumen understand the scope of work as the following:
• Bids include designing and building last mile fiber into each of the five (5) sites detailed in the RFP.
• The initial service delivered to the City of Kalispell will be internet service.
• The City of Kalispell may add or migrate to a private ethernet in the future as your network evolves.
• The City of Kalispell may, or may not order, any of the five (5) sites and the timeline of ordering any
of the locations is not defined.
Experience — Lumen's (formerly CenturyLink, CenturyTel, Qwest and US WEST) roots are in building
networks in Montana and the western states. As the incumbent local exchange provider, Lumen has an
extensive fiber optic and copper network in Flathead County and the City of Kalispell. Lumen has union
technicians in Kalispell who design, build and manage our extensive network. Their expertise is in building
and running networks for our customers.
The Lumen team has an active project for Montana State University Department of Agriculture to build fiber
and deliver Dedicated Internet Access to three (3) of their research facilities — Conrad, MT; Corvallis, MT
and 4570 MT Highway 35 Kalispell, MT. This project is very similar to scope provided in this RFP. The only
difference is their locations are more rural than the City of Kalispell locations and require a much larger fiber
build to reach each location.
b. Identify and discuss any potential difficult issues your company may face in providing services
within Flathead County.
Lumen Response: Our experience in projects of this scope, there are two potential challenges to
expeditiously installing the fiber and network for the city of Kalispell — permitting and weather. Our
construction begins only after all needed permits are issued. The second challenge is weather factors.

LUMEN`
Lumen Response: To limit the possibly project delays due to permitting acquisition, Lumen's service
delivery procedure submits for permits early in the process to ensure each issuing entity has enough time to
review and approve the requested permits. Reviewing this project, Lumen does not foresee any issues with
obtaining all needed permits in a timely manner. Lumen has not experienced delays in any recent projects
due to not receiving timely approval of permits in City of Kalispell or Flathead County but calling this out as a
risk factor is prudent.
Since this project will require boring of fiber as part of the fiber installation, weather could be a factor in
completing the project in a timely manner. Snow on the ground delays the ability to direction bore. With
winter pending, Lumen will schedule our crews to complete any needed boring early in the project timeline.
The overall amount of boring is considered minimal so this work can be completed in a few days.
6. Approach to Contract Management (21 points)
a Describe your company's approach in initiating and establishing the service that meet the needs
and requirements of the City.
Lumen Response: Lumen is a service focused company that has a dedicated support team assigned to the
City of Kalispell.
Lumen's Approach
• Upon RFP award, negotiate an agreement with City of Kalispell that meets any special
requirements of the City of Kalispell.
• Upon agreement signature by both parties, this automatically initiates a Service Order Request in
the Lumen Provisioning system
• Once the Service Order is created in our system, our Project Coordinator, Bethany Cassidy,
manages the order process and tasks through service acceptance by the City of Kalispell. In
addition, she is the single point of contact for the city and provides weekly (more if needed) updates
on the progress of the project. This communicates starts with a project kick-off call with City of
Kalispell key personnel and all Lumen associates involved in delivering the service to ensure all
requirements are captured, roles and responsibilities are defined, and installation timelines are
agreed to.
b. Describe systems used for planning, scheduling, estimating, and managing manpower
requirements.
Lumen Response: In 2020, Lumen deployed Itential Automation Platform (www.itential.com) across our
Field Operations, Service Delivery and Service Assurance which drives automation across platforms for
planning, scheduling, and managing our resources across functional groups. Itential integrates our multi-
vendor environment into single console, seamless workflow for our operations organization. The
deployment of this solution created a single, cross functional workflow process which eliminated any manual
task creations. This enhancement has reduced our intervals in delivering services to our customers.
c. Describe the firm's experience in maintaining training and certifications of personnel.
Lumen Response: Certain job position require professional certifications as a condition of hire while others
offer the opportunity for on-the-job training through the Lumen Learning Platform, Percipio Training Platform,
and through required coursework and testing through the training programs of technology partners like
Cisco, Palo Alto networks, Ciena, AWS, Azure, etc. CyberSecurity Training, ethics training, and leadership
specific training are conducted quarterly and audited for compliance in SAP SuccessFactors

7. Proposal for Project
a. Provide a work schedule with beginning and ending times that meet the scope criteria.
Lumen Response: Lumen will install fiber optic service along with the equipment at each of the five (5)
locations specified in this RFP to deliver Dedicated Internet Access to the City of Kalispell. This project
falls under Lumen's Standard Project Scope and Timelines. Standard Interval for fiber builds and
installation of Internet services is 90 business days

Phase l: Project Planning sets the overall tone for the project. Lumen has learned that developing a
project plan with stakeholder input is vital to smooth project plan execution. Specifically, the project
planning phase entails:
• Identification and definition of all stakeholder and Lumen requirements inclusive of customer
responsibilities and requirements
• Definition and mutual approval of the design and project plan
• Establishment of project team and escalation contact lists
• Addressing inside wiring/point of demarcation extension requirements, if required
• Addressing activation procedures, including circuit testing
• Scheduling and defining the network migration strategy around customer business requirements,
accommodating day-to-day operations by facilitating either of the following options:
o A simultaneous new installation
o A phased, stepped approach to migrating network segments
o A hot cut installation of existing, live network traffic
o installation and testing verification of a parallel network before traffic migration
• Establishing target timelines for the entire project based on the desired due date
Phase II: Service Implementation involves the project's execution and the physical installation of
services. As an initial step, an order is created and quality checked. Accurate and complete service
orders allow us to implement the services with speed and accuracy. We acknowledge receipt of an
accurate and complete order within three days by assigning an order number and communicating via an
order receipt confirmation.
Circuit design also begins at this point. Circuit design requires approximately 24-48 hours, depending on
the complexity of the circuit. The circuit is routed on the network in the most efficient and cost-effective
manner possible. We monitor any network constraint; resolve any design issues, including busy Carrier
Facility Assignments (CFAs) from the access provider; and find creative solutions to potential roadblocks
that could affect final circuit delivery. Within 24 hours of circuit design. we send the access provider an
Access Service Request (ASR), a request between telecommunications companies for interconnectivity.
The day after sending the ASR, we contact the access provider to confirm receipt of the ASR.
Fifteen to twenty business days after the local access provider's receipt of a suitable ASR, a response,
known as a Firm Order Confirmation (FOC), should arrive at our offices. If it has not, we contact the
access provider to escalate a request for an FOC. The anticipated install date for the local access circuit
is established after we receive, from the underlying access provider, the FOC and Design Layout Report
(DLR), a description of how a circuit is engineered.

of Kalispell's preference and budget. Pricing options are attached. Lumen does not break out our CAPEX
costs by line item.
8. Other Factors
a. Current contracts and ability to proceed promptly.
Lumen Response: Lumen's current contracts with the City of Kalispell do not cover the services
proposed in this response. Lumen offers a simple, standard contract for our proposed Dedicated Internet
Access service. Lumen is will int to negotiate any changes to our terms and conditions to meet the needs
of the City of Kalispell.
Lumen's resources are ready to start this project for the City of Kalispell as soon as the contracts/service
orders are executed. All resources Lumen uses to install these services are on -staff and ready to deriver
this solution.
b. Willingness to abide by the City's needs and requirements with few or no objections or changes.
Lumen Response: As a telecom provider who has been serving the state of Montana for over 100 years,
we strive to partner with our customers to meet their individual needs and requirements. Many members
of the Lumen team are citizens of Montana and specifically Flathead County, so we have a vested,
personal interest that we meet and exceed the needs and requirements of the City of Kalispell.
The City of Kalispell will have formal requirements (permits, zoning, etc.) that Lumen has a history of
abiding by. For informal needs and requirements, the key for Lumen to abide by these requirements
starts with open and active communication with the City so we understand your requirements and needs.
Once we understand these needs and requirements, our dedicated support team will work internally to
abide by these requirements with few to no objections and changes. Having a history of building networks
for municipalities in Montana, we don't expect to object to any reasonable requests from the City. A
recent example we have encountered of an issue that may fall under this scope, was during a project to
install fiber for a city, our construction crew needed to bore and disrupted the landscaping on a particular
property (which is common on such projects). Our process is to replace the landscaping to the grade prior
to our project. Our crew did this, but the city was not satisfied with our restoration efforts. Our crew went
out to meet with the property manager to discuss his expectations then worked with him while fixing the
landscaping up to his requirements.
c. Relevant factors impacting the quality and value of service.
Lumen Response: Lumen's service will meet or exceed our internal standards of quality and only when
the service meets our quality assurance guidelines will we consider the project completed and hand off the
service to the City of Kalispell.
One value of having Lumen fiber connected to the City of Kalispell is it provides dedicated access to the
portfolio of Lumen Products and Services - Ethernet, Network Security (DDoS, Ransomware Mitigation,
Web Application Firewalls, etc.), Direct Cloud Connect & Storage (Amazon, Azure, Google, etc.), Unified
Communications (Lumen delivered by Lumen, Microsoft Teams, etc.) These value -add services are
embedded in our platform and designed for easy implementation and management of the services.
