Alta Scope of Work Phase 5Exhibit A-1 Additional Services Phase 5 (Construction) Phase 5 continues the Parkline trail into the construction phase following the bidding phase and selection of the contractor. Task 1: Project Management Task 1.1 General Project Management This task includes general project management activities such as internal coordination, project schedule management and invoicing responsibilities. Task 1.2 City Team Coordination Alta and KLJ team will continue to attend internal coordination meetings with the City. We anticipate these not occurring every week, but averaging 75% of the time during construction season between May 2021 and July 2022. Estimate thirty (30) meetings. Task 10: Construction Phase Task 10.1 PreConstruction Meeting Prepare agenda, administer and record minutes of the pre -construction meeting. The Pre -Con will be scheduled prior to the start of work. KLJ will provide 4 conformed copies of the project manual and construction plan set. (Alta and KLJ) Task 10.2 Design Revisions This task includes general time needed for Alta and KLJ to respond to any issues that arise. This task includes • Minor design changes due to site conditions • Minor design changes due to adjacent stakeholder coordination • Changes due to value engineering Given previous experience with this project we do expect new issues to arise and the need to respond and problem solve during construction. Task 10.3 Construction Administration, Construction Survey Staling and Resident Project Representative (Construction Observation) Alta and KLJ will Provide Basic Services to the City of Kalispell in the role of Construction Administration. This task assumed 10 months (June 21 to November 21 and March 22 to June 22 plus) of active construction plus 3 more months (July 22 to September 22) to assist the City close the project and meet FRA close out reporting. A small amount of time during the guarantee period to final acceptance by the City of Kalispell (2 years after acceptance of project elements). Tasks include: 1. Collect documents for contract between City and Sandry Construction (KLJ). City and Sandry perform final review of all contract documents. 2. Review and evaluate product submittals (shop drawings), Request For Information (RFls), product substitution, and change orders (Alta Planning +Design and KLJ). City of Kalispell I } City of Kalispell 3. Weekly coordination meetings with the City and contractor (primarily KLJ). Additional coordination with landowners, emergency services, MDT, and the public as needed (KLJ). 4. Review monthly contractor payment requests (KLJ). 5. Site visits for Substantial Completion and Final Completion with contractor and City staff (KLJ1. If necessary site visits for close out of warranty period (KLA. 6. Notify and coordinate with the City, contractor and subcontractors when Davis Bacon surveys to be conducted in accordance with grant guidelines (Kl_ y. KLJ will Provide Basic Services to the City of Kalispell in the role of Cdnsction Survey Staking and Resident Project Representative (Construction Observation) for an estimated duration of 9 month (June 21 to November 21 and March 22 to May 22) including but not limited to: 1. General- Provide services to coordinate the work amities of the City, Contractor, and other affected parties, and ensure that work is performed in general conformance with the Construction Documents By performing the services, no authority or responsibility is assumed to supervise, direct, or cones Contractor's work or the Contractor's means, method, techniques, or procedures of construction. 2. Observation- During construction, provide required site observations. The consultant shall verity through observation that all work being performed is pethe Contract Documents. 3. Documentation- The Resident Project Representative shall prepare and keep: detailed notes, computation and measurements, records of quantities of pay items used in the work, and the tests, certifications, or basis of acceptancefrtthese materials and a record of the Contractor's operations as required by federal grant. 4. Construction Staking- Provide construction staking for the prime contractor and their subcontractors KLJ assumes 50 trips or a total of 250 hours to prepare andruiuct construction staking. Task 10.4 Final inspection, Project closeout and Atecorded Plans 1. Substantial Completion Inspection- Upon Substantial Completion, inspect the construction work and prepare a listing of those items to be completed or correetd before final completion of the contract. Submit results of the inspection to the City's Project Manager and the Contractor. 2. Final Completion Inspection- Upon completion or correction of the items of work on the punch list, conduct a final inspection b determine whether the work is complete. Provide written recommendations concerning final payment to the City, including a list of items, if any, to be completed prior to the making of such payment. 3. As -Recorded Plans- Following completion of the construction of the project provide one set of red line drawings to the City's Project Manager. Record drawings will be limited to areas of the project that are within street rights of way and any applicable buried utilities including conduit and electrical, andvill not be prepared throughout the rest of the trail corridor. KLJ will provide the City of Kalispell with utility locations in electronic format so that they may be integrated into the city's GIS system. Task 10 Deliverables • Attendance and facilitation at Pre -Construction Meeting. • Revisions to final bid documents prior to/during construction. • Construction Administration services by KLJ with support from Alta in accordance with Task 10.3. • Substantial completion, final completion, and warranty inspections by KLJ staff. • Record drawings within street rights of way, water and storm system installation, and electrical and spare conduit. • 4 conformed copies and one digital copy of the Project Manual (S.S" x 1 1 ") and Construction Plan Set 01"x171. 2 1 Alta Planning+Design Scope of Work Assumptions: • No new design efforts are included in this scope of work for items or features that have nk*en part of the 100% bid plans. Any design revisions or substitutions will be considered as part of task 10.2. • Irrigation system record drawings will be the responsibility of the irrigation subcontractor. Plans will show lines and connection points. • Full asbuilts of entire corridor including fencing, trail, plaza concrete, etc will not be provided. City of Kalispell May 29, 2021 Amendment #5 to Contract This Amendment modifies the terms and conditions of the Contract for services in connection with Alta Project 00- 2018-118 Kalispell, MT Trail and Complete Street TIGER Project. By signing below, the signatories represent that they are duly appointed representatives of the respective parties to the Professional Services Agreement ("Contract") between Alta Planning + Design, Inc. ("Prime Consultant"), and City of Kalispell ("Client"), dated May 2, 2018 and possess the authority to execute this Amendment.
