Attachments/Justification (list documents supporting change):
The Public Works Department has requested a change to water service to two (2) residences along Whitefish Stage
Road. The addresses are 651 and 653. The residents are currently serviced by a 2" line originating at the intersection
of East California Street and Whitefish Stage Road. The current plan directed the contractor to reconnect the 2" line,
if necessary. The Public Works Department has requested that the two — 3/4" residential water services be connected
to the new 12" water main being installed along Whitefish Stage Road. The existing 2" water line shall be
abandoned in place and the pipe capped at the existing water main. Temporary water service shall be provided to the
homes at 651 and 653 Whitefish Stage Road. Contract price for temporary water service (KAL-3, Item #329) is a
lump sum of $3,691.49. This item represents 20 residential services. A per residential price is $184.575. For two
additional residential temporary services the costs is $369.15. Please refer to the attached revised Plan Sheet W-15
for changes proposed in this change order.
2. After finalizing which monitoring wells must be modified and how with the DEQ, this change order reflects the
differences between the bid quantity and final DEQ approval (1 less abandonment and 2 additional flush mounted) .
Please refer to the attached exhibit and table showing the final monitoring well modifications.
